Below are six copy-ready request emails for onboarding, renewals, reminders, and broker follow-up, plus a subject-line cheat sheet and a checklist every outbound request should include. When the certificate comes back wrong, use COI rejection email templates for itemized deficiency notices.
What must every COI request email include?
COI request: A written instruction from a general contractor to a subcontractor (and their broker) specifying insurance coverage, limits, endorsements, certificate holder language, and submission deadline before mobilization or contract start.
Every request, whether first onboarding or renewal, should carry the same core fields so brokers learn your format:

Who should receive the COI request?
Send the email to the subcontractor's project contact or office manager with an explicit instruction: forward this message to your insurance broker. Brokers produce and sign ACORD 25 certificates; assuming the sub will "handle it" without broker involvement is a common source of delay.
Include a compliance coordinator as the reply-to contact. Use a monitored address, not a no-reply inbox, so brokers can ask clarifying questions without starting a parallel thread with the project manager.

Use predictable subject lines so subs and brokers can search their inbox later:
